Staffing: The Not-for-Profit Difference

The April reporting quarter for Nursing Home Compare’s (NHC) Five-Star Quality Rating System staffing domain includes staffing and census data from facilities’ Payroll Based Journal (PBJ) and Minimum Data Set (MDS) submissions from quarter 4 of 2017. Previously, this data was derived from Centers for Medicare and Medicaid Services (CMS) forms 671 and 672 as a one-time snapshot measure at the time of survey. This brought into question the staffing domain’s accuracy over time.

PBJ data is submitted quarterly by each facility and is due within 45 days of the quarter’s end. NHC’s reporting quarter for July 2018 will use PBJ data from Jan. 1st through March 31, 2018. The submission deadline for this quarter is May 15th, just enough time to ensure that this is the most accurate reflection of staffing in your facility. The MDS census is equally important. Every effort should be made to ensure that all Entry, Reentry, Discharge, and Death Assessments are completed and submitted timely.

The accuracy of the staffing domain is important because it can add or subtract a star from the overall rating. A staffing rating of four or five stars that is greater than the health inspection rating adds a star to the overall rating. A staffing rating of one star subtracts a star from the overall rating.

LeadingAge NY Technology Solutions analyzed the impact of the Five-Star methodology changes on both the Overall Ratings and the Staffing Ratings. The counts are represented in the grids that follow.

The Overall Rating summary: out of all the New York not-for-profit nursing homes impacted, 67 percent benefited from the methodology changes by adding stars to their overall rating.

Proving once again what research has shown, not-for-profit nursing homes are well-known for their staffing. Now, their Star Ratings will mirror that. Updated Five-Star Analysis Reports will be available by early next week.
