Leveraging Your Five-Star Analysis Report

New discharge planning requirements for hospitals go into effect on Nov. 29, 2019. As part of the final rule, hospitals will now be required to share with patients and families how post-acute care providers are performing in areas that would impact the patient’s choice of discharge destination. Performance data includes quality measure rates that are used to calculate ratings in the Five-Star Quality Rating System. LeadingAge member benefits include a quarterly Five-Star Analysis Report. Use the report to identify ways to improve your ratings, and ensure that your local hospitals have your marketing materials, along with your performance data on quality measures.
