Facility Evacuation Planning Application

The Department of Health (DOH) is initiating its annual health care facility emergency planning efforts for coastal storms, floods, and non-coastal storms. Providers (hospitals, nursing homes, and adult care facilities (ACFs)) in New York City are required to review and complete the Facility Evacuation Planning Application (FEPA) no later than March 31, 2021.

The FEPA (previously known as the Facility Profile Application) is a planning tool designed to facilitate the development and maintenance of health care facility evacuation planning information related to evacuation and send/receive arrangements needed to make that possible.

In addition to the Provider Letter, DOH has included a link that enables an individual to register for a coastal storm webinar as well as an updated FEPA Users Guide for New York City providers.
