Transformation Grants Resource

Providers with questions regarding the third round of the Statewide Health Care Facility Transformation Program should email them to Statewide3@health.ny.gov by Thurs., Oct. 28, 2021. Although the application deadline is Wed., Jan. 12, 2022, there will be no opportunity to get questions answered after Thursday’s deadline.

In total, the Statewide Health Care Facility Transformation Program III will distribute up to $208 million in fund awards to a variety of health care providers statewide. The transformation funds are specifically intended to award projects that:

create financially sustainable systems of care;
preserve or expand essential health care services;
modernize obsolete facility physical plants and infrastructure;
foster participation in alternative payment arrangements including, but not limited to, contracts with managed care plans and accountable care organizations;
for residential health care facilities, increase the quality of resident care or experience; or
improve health information technology infrastructure, including telehealth, to strengthen the acute, post-acute, and long term care continuum
