Slides and Recording Available from DOH Transformation Grants Webinar

On Jan. 31st, the Department of Health (DOH) conducted an applicant webinar on the Statewide Health Care Facility Transformation Program Phase II (SHCFTP II). The slide deck for the webinar is posted here, and the webinar recording is available here. Written questions are due by Fri., Feb. 9th, and applications are due March 14th.

Up to $203.8 million in total is available under the SHCFTP II through a Request for Applications (RFA) to health care providers that fulfill or will fulfill a health care need for acute inpatient, outpatient, primary, home care, or nursing home services in a community. At least $47 million of this total amount is available for community-based health care providers, defined as diagnostic and treatment centers, mental health and alcohol and substance abuse treatment clinics, primary care providers, and home care providers.

The objective of the SHCFTP II is to provide grants to health care providers in support of capital projects, debt retirement, working capital, or other non-capital projects directly related to a capital project that facilitate health care transformation activities including, but not limited to, merger, consolidation, acquisition, or other activities intended to create financially sustainable systems of care or preserve or expand essential health care services. DOH has posted an outline of the grant opportunity here, with more details and a link to the full RFA posted on the Grants Gateway website here.

Substantive questions on the solicitation can be emailed up until Feb. 9th to Statewide2@health.ny.gov, with written responses to be posted on or around Feb. 21st. Prospective applicants may submit a letter of interest via the Grants Gateway and email a copy to Statewide2@health.ny.gov. Letters of interest are due by Fri., Feb. 23rd. Submission of a letter of interest is not required, but organizations that do so will receive notifications when updates/modifications are posted, including responses to written questions. Please note that not-for-profit grant applicants are required to register in the Grants Gateway and complete the Vendor Prequalification process for their applications to be evaluated. Further information can be found here.

If you would prefer that LeadingAge NY submit your organization’s question(s) to DOH for response, please send them to us by Feb. 7th. We will continue to keep members posted on the program.
