LeadingAge NY members are reminded that applications for Round III of the Statewide Health Care Facility Transformation Program are due by 4 p.m. on Wed., Jan. 12^th. We urge members to submit their applications two days prior to avoid any last-minute submission issues. Applicants were notified last week that the Department of Health (DOH) issued Addendum #3 to the application. The change incorporates Article 28 residential health care facilities on the Application Cover Page; this provider type had been inadvertently omitted from the page. Members who are applying should make sure that the updated page is in their submitted application. Addendum #3 can be found on the Grants Gateway website.
