DOH Makes Changes in Communication Around Weekly SMART Forms Surveys

(July 12, 2026) LeadingAge NY members expressed concerns regarding the Department of Health’s (DOH’s) communications around SMART Forms surveys. Concerns were voiced about repeated alerts in relation to submission of weekly COVID surveys that were sent broadly and suggested noncompliance when there was still time to submit the report. LeadingAge NY conveyed these concerns to DOH, and they are making some changes this week. They will limit messages to target administrators and the person who last responded to the survey, and the message will be sent on the Tuesday before the report is due. Then, only if it is past due will a message will be sent broadly to the parties that had been receiving all messages. The weekly reports are opened on Wed at 12:00 am each week.

We appreciate these changes, and are pleased that the Department is attempting to address our concerns. We welcome member feedback regarding this approach.
