DOH Issues ADHC Program Survey Report

The Department of Health (DOH) issued the Program Survey Report (PSR) for Adult Day Health Care (ADHC) programs on April 18, 2022. An introductory section must be completed for all open and closed ADHC programs. Open programs are required to continue on and fill out the entire survey. The PSR is to be completed for the time period of reopening to April 12, 2022. The submission deadline is May 15, 2022. Nursing home administrators are required to certify the accuracy of the report. Programs that reopened and have since reclosed should only complete the introductory section.
