HERDS Send-Receive Arrangement Survey Activated for LHV and LI ACFs

After logging into the Health Commerce System (HCS), Adult Care Facilities (ACFs) in the Lower Hudson Valley (LHV) and Long Island (LI)  should access the survey from the HERDS Main Menu and select Send-Receive Arrangement Survey as the Activity name.

Additional information, including contact information for support, can be found within the survey form.

Facilities must complete the survey and submit the data by COB on Monday, Sept. 9, 2013.
