Transformation Grant Update: New FAQs Posted This Week and Extension Provided for IT/Telehealth Grant
Frequently Asked Questions (FAQs) for both the General grant and the Residential and Community-Based Alternatives to the Traditional Model of Nursing Home Care grant under the Statewide Health Care Facility Transformation Program (SHCFTP) Round IV are due to be posted this week. The General grant FAQ will be posted here, and the Alternatives to the Traditional Model of Nursing Home Care FAQ will be here.
In addition, the Department of Health (DOH) announced on March 5th that it will be extending the application due date for the Health Information Technology, Cybersecurity, and Telehealth Request for Applications (RFA) to March 28th at 4 p.m.
