Transformation Grant Funding Available; Deadlines for Questions Approaching!

The Department of Health (DOH) and the Dormitory Authority of the State of NY (DASNY) announced, earlier this month, three opportunities for funding under the Statewide Health Care Facility Transformation Program (SHCFTP) IV. A wide range of providers across the health care continuum, including nursing homes, home care agencies, hospice programs, and adult care/assisted living facilities, are eligible to apply for grants under the following requests for proposals:

  • SHCFTP IV and V for Health Information Technology (IT), Cybersecurity, and Telehealth Transformation: Supports technological and telehealth projects that facilitate health care transformation activities, focusing on cybersecurity, electronic health records, telehealth, and health management tools. A summary is here.
    • Questions due: Jan. 26, 2024, and should be submitted to DOH here.
    • Applications due: March 13, 2024.
  • SHCFTP IV: Supports capital projects and debt retirement and restructuring, working capital, or other non-capital projects directly related to a capital project that facilitate transformation, including redesigning and strengthening quality, pandemic response, or other innovative models of care, to increase access to care and ensure financial sustainability of health care providers.
    • Questions due: Feb. 20, 2024, and should be submitted to DOH here.
    • Applications due: March 26, 2024, 4 p.m.
  • SHCFTP IV for Residential and Community-based Alternatives to the Traditional Model of Nursing Home Care: Supports transformation, quality, and innovation of residential health care services, including, but not limited to, the conversion of a traditional nursing home into separate residential-style homes; improving health outcomes and quality of the patient experience; creating and contributing to a financially sustainable system of care; or preserving or expanding the ongoing availability of essential health care services to the people of the community.
    • Questions due: Feb. 14, 2024, and should be submitted to DOH here.
    • Applications due: April 9, 2024, 4 p.m.

Prospective applicants are cautioned that they must be prequalified in the NYS Grants Gateway or Statewide Financial System (SFS) to apply. This is a document-intensive process.
