NYS Seeking Organizations to Lead Regional Social Care Networks

On Jan. 16, 2024, NYS announced the availability of funds to create and support Social Care Network (SCN) lead entities that will be responsible for the coordination of social care service delivery in each of the state’s nine regions. These SCNs will be integral to the implementation of the Health-Related Social Needs (HRSN) services funded through the recently approved Medicaid 1115 Waiver

SCN lead entities will serve as regionally based organizations charged with building reliable networks of community-based organizations (CBOs) to better enable HRSN screening and the delivery of social care services by CBOs. Awarded SCN lead entities will also work in collaboration with other stakeholders to address HRSN in each region.

Applicants must be a 501(c)(3) non-profit organization with sufficient experience leading CBOs in their region, as outlined by the Request for Applications (RFA).
