Funding is available under three Statewide Health Care Facility Transformation Program grants. The deadline for questions pertaining to the Health IT-Telehealth grant is Jan. 26^th; the deadline for questions on the Alternatives to the Traditional Model of Nursing Home Care grant is Feb. 14^th; and the deadline for questions on the general Transformation grant is Feb. 20^th.

Through its 1115 waiver amendment, the State is seeking lead entities to create and maintain reliable networks of community-based organizations to better enable delivery of social care services.

Funding is available for three opportunities: the standard Statewide Health Care Facility Transformation Program IV; Residential and Community-based Alternatives to the Traditional Model of Nursing Home Care; and Health IT, Cybersecurity, and Telehealth.

Questions on some of the grant application details are due in January.

Application process opens Dec. 1, 2023.

Letters of Inquiry are due Oct. 3, 2023.
