One is for the standard Statewide Health Care Facility Transformation Program IV; the other is for Residential and Community-based Alternatives to the Traditional Model of Nursing Home Care.

DOH has initiated a "batch" disenrollment from partially capitated MLTC plans for long-term nursing home residents effective Feb. 1, 2024. MLTC plans are required to notify the MLTC Technical Assistance Center of any changes in member status.

Providers who received more than $10,000 in Provider Relief Fund or American Rescue Plan Rural funding between July and December 2022 have until March 31^st to report on use of the funds.

Required weekly nursing home reporting of staff member COVID-19 vaccination status has been updated to mirror resident vaccination status reporting.
