Moving Forward Coalition Seeks Nursing Homes to Pilot New Care Planning Tool

(Dec. 2, 2025) The Moving Forward Coalition – a collaboration founded by LeadingAge National, the Institute for Healthcare Improvement, the Johns Hopkins School of Nursing, and the John A. Hartford Foundation – is seeking nursing homes to pilot a newly developed person-centered planning toolkit and training. The Coalition is working with Kansas State University to launch the toolkit in selected facilities.

The toolkit focuses on the resident's goals, preferences, and priorities. The Coalition notes that nursing homes that join the pilot will experience the following benefits:

  • Regulatory Alignment: Evidence-based tools to meet Centers for Medicare and Medicaid Services (CMS) standards.
  • Empowered Workforce: Training and resources that build confidence and skills.
  • Enhanced Resident Experience: Framework that fosters dignity and choice.
  • National Recognition: Join a leading quality initiative.

The Moving Forward Coalition promotes and tests strategies to advance the goals identified by the 2022 National Academies of Sciences, Engineering, and Medicine Report. More information about the work of the Coalition is available here.
