DOH Releases Children's Health Home Draft Request for Applications

The Department of Health, in partnership with the Office of Mental Health, the Office of Alcoholism and Substance Abuse Services, and the Office of Children and Family Services, has released a draft request for applications for providers seeking to serve as health homes for children with complex physical and/or behavioral health conditions. The Department and its partner agencies are soliciting comments on the request for applications by July 30, 2014 at 4 p.m.  In addition, organizations interested in serving as a health home are asked to submit a letter of interest by July 30, 2014 at 4 p.m. The agencies expect to collaborate with stakeholders over the next several months to tailor New York's Health Home model for children.  Enrollment is expected to begin on Jan. 1, 2015.
