Age-Friendly Health Systems Action Community Launches

An Age-Friendly Health Systems Action Community is being launched by the Healthcare Association of New York State (HANYS) to transform the care delivered to older adults in New York State. With support from the John A. Hartford Foundation, the Institute for Healthcare Improvement, the Health Foundation for Western & Central New York, the Fan Fox & Leslie R. Samuels Foundation, as well as the New York State Health Foundation, the program will connect participating teams from across the state to accelerate adoption of the 4Ms — What Matters, Medication, Mentation, and Mobility framework into their practices. It will provide virtual trainings and monthly webinars to test and implement changes that can improve quality of care for older adults and incorporate age-friendly concepts into care delivery.
