DOH Posts Community First Choice Option (CFCO) Provider Lists
The Department of Health (DOH) has posted listings of organizations that provide services that are part of the Community First Choice Option (CFCO) set of benefits. Through CFCO, New York State is expanding benefits available to Medicaid recipients by offering services and supports that have, until now, only been available through waivers. CFCO is scheduled for implementation starting Jan. 1, 2017.
The DOH listings include those providers currently utilized in the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Programs. The lists may include providers already contracting with MLTC plans and Mainstream plans for these or other services. The listings provide address and contact information for the following categories:
