North Shore LIJ MLTC Members to Transfer to Centers Plan

The Department of Health (DOH) has approved an agreement between North Shore Long Island Jewish (LIJ) Managed Long Term Care (MLTC) Plan and the Centers Plan for Healthy Living whereby North Shore LIJ partial capitation MLTC enrollees will be transferred to Centers Plan effective Dec. 1, 2017 or sooner. North Shore LIJ enrollees who choose not to transfer to Centers Plan must make another plan choice for enrollment no later than Jan. 1, 2018.

North Shore LIJ MLTC Plan currently serves approximately 3,000 members in Nassau and Suffolk counties and 2,600 in New York City. With the transfer, Centers Plan is poised to increase its current enrollment of 18,600 to become the largest partial capitation MLTC plan in the state. Centers Plan currently serves 17,600 members in New York City, about 600 in Long Island, and another 500 in Westchester, Rockland, Erie, and Niagara counties.

All enrollees who are transferring to Centers Plan shall receive 120 days continuity of care pursuant to the approved agreement between North Shore LIJ and Centers. All North Shore LIJ enrollees who choose to transfer to another plan shall receive 120 days continuity of care pursuant to DOH policy 17.02, which became effective Sept. 22, 2017.
