MLTC Enrollee Access to Health Homes

Earlier this month the Department of Health (DOH) issued a letter to Managed Long-Term Care Plans (MLTCPs) informing them that the state is requiring MLTCPs to ensure access to Health Homes on a statewide basis. This requirement does not apply to the Program of All-Inclusive Care for the Elderly. In the letter DOH recommends that MLTCPs execute Administrative Health Home Service Agreements (ASAs) with as many Health Homes in their service areas as is practical to achieve this access requirement. To facilitate formalizing such arrangements, OHIP developed the four documents below:

Statewide Administrative Health Home Services Agreement Between Managed Long Term Care Plan and Health Homes;

Statement and Certification;

Guidance for Providing Care Coordination and Management to Medicaid Members Enrolled in MLTC Plans and Health Homes; and

Care Planning and Coordination for MLTC Plans and Health Homes form.

The DOH letter is available here.
