DOH to Present Webinar on Helping Individuals Transition to the Community

The Department of Health (DOH) is inviting health plan leadership and staff to a webinar that will provide an overview of the Money Follows the Person (MFP) program and an introduction to the MFP-funded Open Doors program, both of which are designed to help individuals transition from facility-based care to the community. Attendees will learn how enrollment and transition of services works via Care Management as members transition from intermediate care and nursing facilities into community settings to receive home and community-based services (HCBS). DOH strongly encourages Managed Long Term Care (MLTC) Care Managers and staff to attend the sessions. The presentation will provide information on how Open Doors’ Transition Specialists can work collaboratively with MLTC/Mainstream care managers and staff and providers to meet the State’s Olmstead obligation to support individuals to live in the most integrated settings.

As the New York State-designated Local Contact Agency for Minimum Data Set (MDS) Section Q, the Open Doors Program assists individuals living in nursing facilities and intermediate care facilities to transition into community settings to receive HCBS in their community of choice. Open Doors also provides outreach and education to nursing homes regarding community transition, Section Q, and nursing homes’ responsibility to refer to the Local Contact Agency (Open Doors).
