Evidence-Based Health Promotion in New York State

The Center for Excellence in Aging & Community Wellness is the Quality and Technical Assistance Center (QTAC) for evidence-based health promotion programs, such as the Chronic Disease Self-Management Program (CDSMP), across New York State.  CDSMP, a six-week workshop series developed by Stanford University's Patient Education Research Center, empowers older adults and their caregivers to take charge of chronic conditions such as arthritis, heart disease, etc. 

Research has shown that participants:

  • are able to manage their symptoms better.
  • communicate more easily with their doctors and loved ones.
  • feel better.
  • are less limited by their illness.
  • spend less time at the doctor or in the hospital.
