Next Phase of UAS-NY Implementation Begins June 1

DOH is conducting a webinar on the UAS-NY to give critical information to those providers in the next phase of the transition plan.  The counties include Cayuga, Chenango, Cortland, Delaware, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, St. Lawrence, Tioga and Tompkins counties.  The provider types within those counties that should participate include:  Adult Day Health, Assisted Living Program, Care at Home I/II Waiver, Consumer Directed Personal Assistance Program, Long Term Home Health Care Program, Nursing Home Transition and Diversion Waiver, Personal Care Services Program and Traumatic Brain Injury Waiver.  In addition, if you contract or plan to contract with any of those providers or a managed long term care plan, you should discuss what steps you need to take to become prepared to conduct assessments.  The process takes a good amount of time and subcontrators should be involved early in the process.  Click here for more information.
