The Department of Health (DOH) announced on Mon., June 30 the upcoming transition of Medicaid-eligible individuals receiving 120 days or more of community-based long term services into Managed Long Term Care (MLTC) in Greene, Saratoga, Schenectady and Washington counties. According to DOH, the mailing of announcement letters to the fee-for-service population will begin this week and the mailing of mandatory letters will begin during the week of July 14, 2014.

DOH reports that, beginning July 7, participants seeking community-based long term care services will be directed to New York Medicaid Choice for information and education regarding MLTC options in their county.  Consumers can access Medicaid Choice at 1-888-401-6582.

Questions regarding this update should be directed to DOH at MLTCTRANS@health.state.ny.us.
