2017 LHCSA Statistical Report Form and Registration Now Available
The 2017 Licensed Home Care Services Agency (LHCSA) Statistical Report is now available on the Healthcare Financial Data Gateway on the Health Commerce System (HCS). The report includes a newly established registration requirement pursuant to state budget language enacted this year. Please note that this report is in a new format and will require extra time and attention. The budget language also establishes penalties for LHCSAs that fail to complete this report. The submission deadline for the 2017 LHCSA Statistical Report is Nov. 16, 2018.
The following documents are available on the HCS to assist you:
- Instruction document on downloading and installing the Report Manager software and downloading the report.
- Instruction document that reviews all forms to be used for the 2017 LHCSA Statistical Report.
- Frequently Asked Questions document, which will be updated and distributed multiple times during the reporting period.
Please read the documents carefully. Instructional training has been recorded and is also available on the HCS. The training has been broken into five parts:

- Part V Validate, Finalize and Upload the Report
