Updates on Three Home Care Waivers - LTHHCP, NHTD, and TBI

Long Term Home Health Care Program (LTHHCP) - The Department of Health (DOH) has confirmed the Centers for Medicare and Medicaid Services (CMS) approved on Feb. 24th, the third temporary extension of the LTHHCP. This means the LTHHCP is operational until May 27, 2016. Numerous LTHHCP have closed, and at this time there are fewer than 100 patients left on the waiver. The waiver cannot be “taken down” until all of patients have been safely discharged.

Nursing Home Transition and Diversion (NHTD)/Traumatic Brain Injury (TBI) Transition Workgroup - The NHTD and TBI transition workgroup continues to meet weekly to discuss phase one issues. Phase one issues are relevant to DOH’s transition plan to CMS. Phase two issues are pertinent to the transition of the waivers to managed care scheduled for Jan. 2017.

The phase one issues have included:

  • Access to services, what services should be included in the managed care benefit plan, what should remain as fee-for-services, and what services should be carved out. An example of a service that will be carved out is home visits by medical personnel;   
  • Changes to service definitions as a result of CMS approval for the Community First Choice Option;
  • Transition timelines;
  • Provider qualifications;
  • Enrollment of existing waiver participants;
  • Enrollment after the transition;
  • Initial plan assessments for transition waiver participants;
  • Notification requirements;
  • Plan readiness to accept existing waiver participants;
  • Continuity of care provisions;
  • Appeals and rights; and
  • Quality assurances.

DOH posted to the MRT #90 website numerous hand-outs and mins. from the transition meetings. The latest hand-outs have been twelve Frequently Asked Questions (FAQs), crosswalk of NHTD/TBI provider qualifications, and a crosswalk of Service Coordination with the plan's care manager role.

This week meeting on Thurs., Mar. 3rd from 1:30 - 3:30 p.m., will be at One Commerce Plaza (99 Washington Ave.), 16th floor, Room 1613, Albany, NY 12210. The meeting will continue the discussion on the role of a Service Coordinator with  the plan’s care manager.
