Grant Opportunities for Community-Based Organizations and Providers

Department of Health (DOH): Healthy Neighborhoods Preventive Health Cornerstones – The program is designed to provide preventive environmental health services to targeted geographic areas. An estimated $19.6 million in funds is available. Applications are due Sept. 13th. Click here for more information.

New York City Department for the Aging (NYC DFTA): Caregiver Services Program-Manhattan I – The Caregiver Program is designed to support the giver of care and the receiver of care so that the relationship can thrive in the midst of changing roles and experiences. Staff from the Caregiver Program provide individualized information and assistance with accessing benefits, supportive counseling, and training to caregivers seeking help. Caregivers can also access support groups, respite care, and supplemental services through the program. Entities providing any of the following services may apply for funding: caregiver support, case management, academic supports, child care, parenting services, respite care, home attendant services, or homemaking services. Anthem Foundation Program Grants – Anthem provides funding to support ongoing community health programs that address the social determinants of health and positively affect conditions identified through their signature Healthy Generations program. Applications for funding to support community health programs are currently being accepted and are due by 5 p.m. on Aug. 9th. Click here for more information.
