Home Health Change of Care Notice (HHCCN) is Available

In June 2016, the Home Health Change of Care Notice (HHCCN), Form CMS-10280 was revised and approved by the Office of Management and Budget (OMB). The revised HHCCN form that Home Health Agencies (HHAs) use to notify Original Medicare beneficiaries receiving home health care benefits of plan of care changes can be accessed at https://www.cms.gov/Medicare/Medicare-General-Information/BNI/HHCCN.html

This form replaces the Home Health Advance Beneficiary Notice of Noncoverage (HHABN). The HHABN will be discontinued in the near future. Home Health Agencies (HHAs) may begin using the HHCCN as soon as possible, although according to the Centers for Medicare and Medicaid Services (CMS), the effective date for the revised form is 90 days from the CMS announcement, which was issued on Oct. 9th.

Currently, Medicare requires HHAs to issue notification to Medicare beneficiaries prior to:

  • providing care that Medicare usually covers but may not pay for in instances because the care
    • is not medically reasonable and necessary;
    • is considered custodial care; or
    • is not covered because the beneficiary is not homebound;
  • changing care for HHA reasons; and/or
  • changing care because of physician’s or provider’s orders.
