Institute for Healthcare Improvement to Launch Second Age-Friendly Health Systems Action Community

The Department of Health (DOH) informed LeadingAge NY last week that the Institute for Healthcare Improvement (IHI) is launching their second Age-Friendly Health Systems Action Community and is seeking additional participants.

This Action Community will begin on Mon., April 1st. It is a free, seven-month learning community that succeeds the first Action Community of teams from more than 75 systems across the nation working together to rapidly test and scale the Age-Friendly Health Systems 4Ms (What Matters, Medications, Mentation, and Mobility) Framework. Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and IHI in partnership with the American Hospital Association and the Catholic Health Association of the United States. For more information about the initiative, click here.
