DOH Announces Finalists in Social Determinants of Health ‘Call for Innovations’ Initiative

Last week, the Department of Health (DOH) announced the finalists of its Social Determinants of Health ‘Call for Innovations’ Initiative. The nine finalists, representing community-based organizations, health care providers, and technology companies, were selected from over 200 applicants from across the country, “all of whom have come up with creative and effective ways to address social determinants of health for Medicaid members.” Social determinants of health are defined as the structural conditions in which people are born, live, grow, work, and age, and addressing them can have a significant impact on health outcomes at a much lower cost than medical interventions.

DOH has invited the nine finalists, including LeadingAge NY members ArchCare and VillageCare, to present their innovations to a panel of expert judges and audience members at a Social Determinants of Health Innovation Summit on Wed., Sept. 26, 2018. Finalists will not receive funding or monetary awards from the State but will have the opportunity to hear and share best practices in the field and receive exposure to key health care decision makers.
