Transitional Adult Home Compliance Plan Extended Again

The Department of Health (DOH) has again extended the deadline for submission of the Transitional Adult Home Compliance Plan. According to the alert on the Health Commerce System, the new deadline is Dec. 1, 2014. The Compliance Plans should be submitted to DOH via email at transitionalah@health.ny.gov. Questions should be submitted to DOH's Community Transitions Program at the same email address.
