Nursing Home and ACF Members Must Review Critical Asset Survey in HERDS

The Department of Health (DOH) sent a message this morning informing adult care facilities (ACFs), nursing homes, and hospitals that the Critical Asset Survey in HERDS has been reactivated for the first quarter 2019 review/update.

The message reports that the three forms of the Critical Asset Survey (Baseline/Surge/Resilience) have been merged into one combined form going forward. All data that was previously submitted in both the Surge and Resilience forms has been migrated to the new combined form and therefore does not need to be re-entered.

The Critical Asset Survey review and update period runs from Jan. 1-March 31, 2019. All ACFs, nursing homes, and hospitals are required to review their previously submitted data, update any that has changed, and (re)submit the Critical Asset Survey data during this time period.
