DOH Asks ACFs to Submit Data to Assist in Updating Database

The New York State Department of Health (DOH) Division of Adult Care Facility/Assisted Living (ACF/AL) Surveillance is updating its ACF database and will be collecting the Taxpayer/Employer Identification Number (TIN/EIN) for all facility operators. This information is needed to transition to a new system, currently in use by other divisions, to electronically generate operating certificates. This platform is known as the Health Facilities Information System (HFIS). The requested TIN/EIN information will NOT be accessible to the public nor will it print on a facility’s electronic or printed operating certificate.

LeadingAge NY has been asked to assist in ensuring that DOH has your TIN or EIN and asks that you send that information to acfcon@health.ny.gov with the subject line ACF Data Collection by close of business on Nov. 20th. Questions regarding this effort can also be directed to that DOH email.
