DOH Announces Change to Part II Review for Application for ACF and Assisted Living Licensure

The Department of Health (DOH) has issued a Dear Administrator Letter (DAL) announcing a change in the process for the Part II review of any application for adult care facility (ACF) or assisted living licensure. Effective April 2, 2018, documents previously reviewed onsite during the pre-opening or first scheduled survey (ACF Common Application, Schedule 6- Page 5) must be submitted as part of the complete Part II application package. DOH has revised Schedule 6 of the ACF Common Application to reflect this change and anticipates that this will help to expedite the review process.
