ACFs Are Reminded to Complete QSIR by Oct. 31st

Adult care facility (ACF) and assisted living members are reminded that the 3rd Quarter Statistical Information Report (QSIR) must be completed and submitted by all ACFs via the Health Commerce System (HCS) by Oct. 31, 2018. The Department of Health (DOH) does not accept late submissions; failure to submit this report by the due date may result in enforcement action. Additionally, facilities with a certified bed capacity of 80 beds or more, in which 20 percent or more of the resident population are persons with serious mental illness, are required to submit additional information outlined in regulation.

Please note that while several people in the ACF’s organizational roles may input data for this report, ultimately the Administrator must review the report prior to submission, complete the attestation statement, and submit the completed report.

Click here to view the Dear Administrator Letter (DAL), which provides additional details as well as the directions on where to access the report on the HCS. Click here to view the Definitions of Terms and here for Data Entry Instructions.

Questions pertaining to this DAL and programmatic requirements should be directed to DOH’s Georgina Raus or Jillanna Devik at 518-408-1133. Questions specific to the Roster of Adult Home Residents should be directed to DOH’s Matthew Gasbarro at matthew.gasbarro@health.ny.gov.
