DOH Activates ACF Quarterly Statistical Information Report on HCS

The Department of Health (DOH) has activated the Adult Care Facility (ACF) Quarterly Statistical Information Report (QSIR) on the Health Commerce System (HCS) for the first quarter of 2017. All ACFs must submit the report. Additionally, facilities with a certified bed capacity of 80 beds or more, in which 20 percent or more of the resident population are persons with serious mental illness, are required to submit the Roster of Adult Home Residents. The QSIR and Roster of Adult Home Residents, if applicable, are due no later than April 30, 2017. Please note that the Department has issued violations for failure to submit the QSIR on time.
