Reminder: DOH Has Activated ACF QSIR for First Quarter of 2023
The Department of Health (DOH) has activated the Adult Care Facility (ACF) Quarterly Statistical Information Report (QSIR) for the first quarter of 2023. All ACFs must complete the report, and it is due by April 30th.
DOH issued this Dear Administrator Letter (DAL) outlining the requirement. DOH also provided two enclosures to guide in the submission of the report:

- Enclosure 2: QSIR Slide Deck
Members are reminded that failure to submit this report on time could result in enforcement action.
