ACF Financial Report – Updated Enclosure Documents

Last week, LeadingAge NY reminded adult care facility (ACF) and assisted living members that the due date for the 2021 ACF Annual Financial Report is approaching. We wanted to note that two of the Enclosures have been updated by the Department of Health (DOH) since the original posting of the materials:

• Independent Report of Certified Public Accountant (CPA) Single Facility – Enclosure II
• Independent Report of CPA Consolidated Financial Report – Enclosure III

To verify that you are using the proper form, the bottom of the last page should indicate that the form was developed in January 2023. Click here for more information regarding the ACF Financial Report.
