ACF 3rd Quarter Statistical Information Report Due by Oct. 31, 2017

The Department of Health (DOH) has activated the Quarterly Statistical Information Report (QSIR) for the third quarter of 2017. All adult care facilities (ACFs) are required to complete the report, which encompasses the time period from July 1, 2017 to Sept. 30, 2017. Additionally, facilities with a certified bed capacity of 80 beds or more, in which 20 percent or more of the resident population are persons with serious mental illness, are required to provide additional information, which is outlined in the Dear Administrator Letter.

The report must be submitted on the Health Commerce System (HCS) by Oct. 31, 2017. Failing to meet this deadline could result in enforcement action and the imposition of civil penalties.
