Palliative Care Access in the Community: Home Care and Assisted Living Facilities

A free webinar entitled, Palliative Care Access in the Community: Home Care and Assisted Living Facilities, has been schedueld for Thurs., Feb. 19, from 3 p.m. to 4 p.m. This webinar will explore the key elements of establishing and strengthening palliative care service partnerships and integration models in home care and assisted living settings, and how to apply palliative care principles to the care of residents and their families. This webinar will include:

  • the importance of extending palliative care into the community;
  • models of community palliative care delivery in the home care setting;
  • the importance of palliative care and hospice services in assisted living facilities; and,
  • evidence-based outcomes of community-based palliative care.
