DAL Outlines Some Flexibility in How the ALR Medical Evaluation Form is Completed

ABSTRACT: Department will allow other professionals to complete certain aspects of form.

The Department of Health (DOH) has issued a Dear Administrator Letter (DAL) on the Health Provider Network (HPN) regarding the completion of the revised Medical Evaluation form (DOH 3122) for use in Assisted Living Residences (ALRs).  While the Medical Evaluation remains lengthy, the DAL outlines some flexibility in how the form can be completed.  NYAHSA has been advocating for increased flexibility regarding the completion of the ALR forms, and are pleased that the department is allowing some degree of flexibility.  

The DAL and the revised Medical Evaluation can be found on the ACF page of the HPN.  For your convenience, copies of both are provided as attachments at the bottom of this screen.
