State Seeking Engagement on 1115 Waiver Activities

(Nov. 12, 2025) The Department of Health (DOH) Office of Health Insurance Programs (OHIP) issued a communication last week to Chief Executive Officers (CEOs) and hospital administrators urging their engagement with the six Social Care Networks (SCNs) to assess individuals for Health-Related Social Needs (HRSN) under the 1115 Waiver.

LeadingAge NY has outlined the 1115 Waiver initiatives in past Intelligence articles here and here regarding SCNs and the State's effort to address underlying social determinants of health of Medicaid recipients.

We ask members to consider engaging in this effort. Providers with home care agencies and care navigation service lines are experienced in this work and are ideal candidates to provide HRSN assessments. Providers should connect with the SCNs to partner in this effort.
