State Announces $300M in Statewide Healthcare Transformation Grant Awards
(Aug. 12, 2025) On Aug. 8th, New York State announced nearly $300 million in Statewide Healthcare Transformation Grant awards to 55 organizations across the state aimed at expanding access to inpatient, primary, behavioral health and long-term care services. Those who applied for funding under the main transformation program funded at $250 million, as well as those seeking some of the $50 million targeted for developing alternatives to the traditional model of nursing home care, were notified last week whether their applications were successful or not. While hospitals and diagnostic and treatment centers comprise the largest group of funded organizations, at least 15 projects (27% of the awards) include some long-term care focus. Initiatives receiving funding include small home and household-based nursing home projects, Adult Day Health Care and PACE, construction or renovation at several Adult Care Facilities, expansion of pediatric long term care capacity, as well as a plan for an innovative dementia village. Members should be aware that these awards account for two of the three Statewide Transformation Program Requests for Applications (RFAs) that were issued in early 2024. Awards for Health Information Technology, Cybersecurity, and Telehealth Transformation projects (RFA# 20258) have not been announced yet. While the timing and total amount to be awarded are not yet known, State Budget allocations for technology-oriented Transformation Grants totaled $650 million.
