State Opens 2025 Cost Report for NHTD and TBI Waivers
(Dec. 9, 2025) The Department of Health (DOH) opened the 2025 Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Program Cost Report process by sharing its 2025 Cost Report manuals and templates. Providers must complete and submit their Cost Reports by April 30, 2026. DOH made a few changes to the templates and reporting requirements which are summarized on page 2 of the manual and discussed more in each respective section.

DOH will be holding a webinar to review the 2025 NHTD/TBI Cost Report process on Wed., Jan. 14, 2026 at 1 p.m.
The Department will provide a review of the manual and templates and will address questions providers may have. Please review the templates and instructions and save any questions for the day of the webinar. Feel free to forward to whomever needs to be on the call.
