Funding Opportunity for Healthy Families Home Visiting Program

The New York State Office of Children and Family Services (OCFS) announced a funding opportunity aimed at providing quality home visiting services through the Healthy Families New York (HFNY) home visiting program in some of the highest need communities in New York State; where intensive home visiting services are unavailable. HFNY is a voluntary home visitation program for expectant parents and families with infants which promotes optimal prenatal care, better birth outcomes, and positive growth and development in order to improve parent child bonding, maternal and child health and social outcomes for families at high risk of abuse or maltreatment and poor health outcomes. The program provides intensive home visitation services bi-weekly during the prenatal period, weekly until the child is at least six months old, and periodically thereafter based on the needs of the family until the child begins school or Head Start.

As outlined in the Request for Proposal (RFP), a total of $1M will be available and the funding will provide an opportunity to begin new or expand existing HFNY home visiting programs in high-need and underserved communities. Additional consideration and points will be awarded for proposals in areas where intensive home visiting services do not already exist. If you are interested, please review the 2011-2013 New York State vital statistic data in order to determine the geographical need with regard to issues surrounding pre-mature births, low birth weight, out of wedlock births, Medicaid births, infant death rates, late or no pre-natal care, neonatal death rate and the teen birth rate. Pages 15 and 16 of the RFP outline the program requirements and outcomes. On page 17, it outlines several additional program components, individually or in combination, which may be included in the application proposal.

The RFP time table includes, submittal of question deadline - June 10, 2016; date of answers - June 17, 2016; and applications are due by July 8, 2016 by 3 p.m. eastern standard time.

The contracts awarded to the HFNY Home Visiting program as a result of this RFP will be for a five year term, subject to successful renewals on an annual basis.
