DOH Prepares 2018 CHRC Reimbursement for ACFs, LHCSAs, and Hospice Providers
The Department of Health (DOH) is preparing to issue reimbursement checks for 2018 Criminal History Record Check (CHRC) fingerprinting for all Adult Homes, Enriched Housing Programs, licensed home care services agencies (LHCSAs), and Hospice providers and has issued a Dear Administrator Letter (DAL) requesting that all providers review and, if necessary, update their entity’s information on the New York Statewide Financial System (SFS). According to DOH, several providers either do not have an account with the SFS or their account contains inaccurate information. Please review and update your account via the portal link provided in the DAL by Fri., March 20th. The Department will be unable to remit payment to providers without an SFS account or those who have an account with an incorrect address/location. DOH encourages you to enroll in direct deposit through the portal as well.
Providers who are not on the SFS must immediately register by completing the New York State Office of the State Comptroller Substitute Form W-9: Request for Taxpayer Identification Number and Certification. Additional details on that process are provided in the DAL.
