DOH Offers Person-Centered Planning Training

The Department of Health (DOH) has asked LeadingAge NY to ensure that you are aware of a training initiative to assist with the implementation of person-centered planning. Please see the below announcement.

The NYSDOH is excited to announce the Person-Centered Planning Comprehensive System Transformation Statewide Training Initiative. This initiative is sponsored by DOH to support best practices in Home and Community Based Services (HCBS) Final Rule-compliant person-centered planning and practice throughout the State.

What: The training initiative will consist of an extensive series of Regional Trainings and Learning Institutes held across the State at no cost to training participants. Regional Trainings will consist of full- and half-day trainings, led by instructors certified in Person-Centered Planning/Plan Facilitation by The Learning Community for Person-Centered Practices (TLCPCP). Trainings on Person-Centered Plan Development, Person-Centered Plan Implementation, Person-Centered Practice for Managers and Person-Centered Thinking Train-the-Trainer will be offered. Learning Institutes are 4- to 5-month facilitated and self-directed learning journeys, which include 4 in-person training days as well as webinars, coaching circles and other learning opportunities. They are unique learning experiences grounded in Theory U, a perspective and set of practices developed at MIT that support transformational change and inform approaches to person-centered planning and practice.

When: Beginning in Spring 2019 through the end of 2020. There will be 80+ Regional Trainings and six Learning Institutes held each year. 

Where: Each training opportunity will be offered multiple times in every region of New York State. 

Why: To grow best practices in person-centered thinking, planning, and practice capacity in HCBS covered service systems in the State, and to increase HCBS Final Rule compliance in person-centered practice.

Who: These training opportunities are intended for a variety of audiences around the State, including, but not limited to:

  • Agency and plan managers
  • Staff involved in service-planning, service delivery, and direct-care
  • Care managers, case managers, or service coordinators
  • Individuals receiving Medicaid-funded HCBS and their circles of support

New York State’s HCBS for individuals with disabilities and disabling conditions, including physical, behavioral, developmental, age-related, or cognitive/intellectual, take place in a variety of settings such as private homes, group homes, scatter site apartments, adult care facilities, day habilitation sites, adult day health care centers, and social adult day programs. Regardless of where individuals receiving HCBS live or receive services, or through which service system they are accessed, all HCBS recipients are required to have Person-Centered Service Plans/Recovery Plans/Resident Care Plans/Life Plans/etc., that afford them informed choice of services, supports, and residences in compliance with the Person-Centered Planning standards of the HCBS Final Rule. Participation in training through this Statewide Training Initiative can demonstrate efforts to come further into alignment with these standards.

Below is the registration link, which in addition to the registration form includes a summary, agenda, faculty descriptions, and location information. As part of the registration process, we ask that you please fully complete the Invitee Questionnaire. The purpose of this questionnaire is to gather valuable information about your background and interest in person-centered planning and intent in participating in this Learning Institute. Your answers will inform the Learning Institute faculty as they tailor the curriculum for this experience.
