Further Modifications to Training Schedule for ACF/Assisted Living Application

LeadingAge NY is very happy to announce that the streamlined application process for all categories of Adult Care Facility (ACF) and assisted living is now available. As members may recall, LeadingAge NY worked with the Department of Health (DOH) and other stakeholders to update, streamline and consolidate the various applications. In addition, we developed additional streamlined options that will expedite certain types of projects. After many months of work, the final products are now available for use, and DOH will be conducting training across the State to help providers understand the options.

Training Information:

The DOH training on the Common Application is free, however, seating is limited and registration is required.  It is important to note that several details about the trainings have changed since we announced these opportunities, including changed dates and consolidation of trainings.  Please review the below carefully. More Information About Streamlined Processes

The streamlining initiatives include a variety of streamlined processes and an Adult Care Facility Common Application. The Common Application will be used to apply to become newly licensed, and to make changes to an existing ACF license. Revisions include, but are not limited to, change of operator or renovation and/or expansion of an existing facility. In addition, several forms were created to promote the expeditious review by DOH of applications that meet certain criteria. The ACF Common Application and other streamlined forms may be used immediately. We anticipate that applicants will be able to submit an application electronically in the near future.

Another process that was developed as a part of this effort is the Operator in Good Standing process, designed for known applicants who are licensed operators in good standing and who have submitted and received approval on an application of the same type and size approved within the last two years. Pending applications previously submitted by applicants that meet the criteria of “in good standing” as defined by SSL 461-b (2) (b) may revise their applications to take advantage of the streamlined processes.

LeadingAge NY is extremely proud of these significant accomplishments, which are the result of the dedication of resources, significant efforts and the spirit of collaboration among the work group partners. We hope that this will ultimately be a benefit to providers, DOH and to seniors who need these services.

The revised Common Application and related documents are available on the Health Commerce System, as well as the public site, here. Click here for the most current PowerPoint presentation and here for the Dear Administrator letter issued by DOH. All inquiries regarding the Common Application should be forwarded to DOH at: acfcon@health.ny.gov. Questions regarding existing applications should be directed to your DOH Project Manager.
