DOH Issues Updated ALP Medical Evaluation

DOH has issued a Dear Administrator Letter (DAL) announcing revisions to Medical Evaluation (DSS-4449C) for the Assisted Living Program (ALP).  In mid-June, DOH had issued an initial revision to the form, which had been updated given the implementation of the UAS-NY.  DOH received feedback about the changes, and has issued a newly revised form.  As we move to the new UAS-NY assessment processes, it is possible that new issues may be discovered that need to be addressed, so we urge members to review the DAL and revised form carefully, and provide us with feedback. We are in regular contact with DOH regarding UAS-NY implementation, as well as assisted living matters, and will communicate any outstanding needs or concerns.
