DOH Issues Revised Closure Guidelines

The Department of Health (DOH) has issued revised guidelines for providers seeking approval to close a facility or discontinue programs that provide medical care and/or other services to individuals in community-based, residential or acute care settings.  This includes hospice, Adult Care Facilities (ACFs), nursing homes and hospitals.  Click here to view the guidelines.
